Is it a requirement when changing to 5 day thawed plasma to have a disclaimer on the new product label that states this is not an FDA approved product?

Thawed Plasma is not recognized by the FDA so is not a licensed product and should not contain an FDA license number.  If you make this product, all you would need to do is remove any license number(s) from the label.
