Hopefully I've added the poll correctly.  But I'm curious to know, upon activation of MTP do you set up O Positive PRBC's or O Negative?  These are uncrossed without knowing the patient blood type.  We would move to type specific as soon as possible.

Thanks,

Michael

85% of those unknown types will turn out to be Rh positive so O pos will do them no harm.

  • Magnum, I'm just currious how many times your MTP notification states whether or not the patient has a uterus!

  • In many cases we don't know if the patient is male/female or the age so we'll start with O neg. Then we'll make informed decisions after information is available.

Would start with O, D Positive every time.

You don't know what else is coming in, so save your O D Negative for a woman of child bearing potential (sorry, that should be female - may be a young child).

Most of the reactions will be ex vivo (on the floor) if they already have an anti-D, and if they don't, does it matter if they make one?

Exactly Malcolm... Too many possibilities to deny blood in an emergency situation just because it isn't 'perfect'.

O POS until type is known.

Females <50 y.o. will get O Neg.

We usually start with 2 units of O Neg, but we move to type specific as soon as we have a qualified Blood Bank specimen (banded, correctly labeled).  If unable to obtain a Blood Bank specimen, we would switch to O Pos within 4 units.

If the patient is O Negative, we will start with O Negative and give up to 6 units (if available).  After that, we will switch them to O Pos.  If the patient is not O Negative, we will give O Negative until we get a solid type and can swith to group specific, or if our O Negative supply is running low, we will get a pathology deviation approval.

We would start with 2 ONeg, then switch.  Hopefully have a patient type by then.

we start with O Pos for males, females >50 years of age, and women without a uterus.

Magnum,

I'm just currious how many times your MTP notification states whether or not the patient has a uterus! :)

Because there are now people who identify as male yet have a uterus (and some have used said organ to produce children), maybe we should have that information included in our notifications.  That is really the salient point rather than their assigned gender.  (I am not having any luck coming up with a proper emoticon to not come across as ridiculously serious about this but not seem to be making fun of transgendered people either.)
