We have been using a transfusion slip (single page) that has a Blood Bag label ( with acceptable glue) on the form.  We then peel the Blood Bag label off the transfusion slip and place it on the units.

Our supplier (Staples) is having trouble getting an acceptable label going forward.  I was wondering if other places are using somethings similiar and if so- who is your vendor?  Didn't get much notice about the product being unavailable so need help asap.  Thanks.

We used stick on lables for many years and then found that they would occasionally fall off the bags after they were hung.  This happened most often with our bags of platelets for some reason (different plastic?). 

 

Anyway, we now use a two-part tag with the back 1/4 sheet having the basic info on it.  Both are attatched to the unit with a gun.  Only the full tag is removed at the bedside.  This system is cheaper than the old sticker/tag combo we used to use.

we use a stick on label that we wrap around the "tail" of the bag. That way it is sticky side to sticky side and not to plastic. Can't fall off.

We used to use a form with a peel off label similar to sblanchard.  The label had an FDA-approved adhesive for use on blood bags, but it didn't always stick well to cold plastic.  We went to a system like  Auntie-D describes.  We print onto a label.   One label sitcks on a tie tag and a 2nd label is attched to a pre-printed Transfusion Record form.

Same as Auntie-D. We have stock 8 1/2 x 14" paper with a perforation on the bottom, so we end up with a 8 1/2 x 11" transfusion record form and a bag tag.

For those using labels - they are approved I hope?

Yes, when I ordered them years ago they had the same glue from 3M that is used for base labels on blood bags.. Hope they haven't changed!

Check with your manufacturer to make sure you didn't get a new lot with the wrong glue. That happend to use a year or 2 ago. The labels weren't sticking and when they checked with manufacturer the wrong glue had been used.

We use bag tags that we tie on the units. As far as I know we can not put any lables directly on the blood bags. This is a regulation.
