You have to be able to identify the phlebotomist - how you do it is up to you.  Many regulatory agencies require this documentation on the specimen.

AABB Standards, 28th ed, 5.11.2.2 There shall be a mechanism to identify the date of sample collection and the individual who drew the blood from the patient.

So there is no regulation that states it has to be a signature on the specimen itself.

Hello everyone- It is a CAP requirement TRM.40230 All blood samples used for compatibility testing are labeled at the time of specimen collection in the presence of the patient with:

1. Patient's first and last name
2. Uniques identification number
3. Date of collection
4. Identity of the phlebotomist

Besides all of the above, I believe in the US, the federal CLIA statute requires a facility to be able to identify who collected a specimen - be it a tumor biopsy, blood or urine. This would be hard if no one is required to ID themselves by some way with the specimen itself.
