I  need assistance on the best way to document the quantity of mL to issue to a neonate.

Not sure - do you just sign out an entire unit and the transfusionist takes what they need and discards the rest?  Or do you actually divide the unit into pedi packs?  If you divide, there is an option in soft to Create Product and Divde.  You would change the quantity at that time.

 

(You can also change the quantity when signing out the product)

 

Otherwise you can edit the transfusion after the fact to change the quantity transfused (useful in cases where only 1/2 of the unit was infused).  Patient - transfusion - edit (only on units that have either been confirmed transfused or presumed transfused.)

We document it on the Transfusion Record that accompanies the syringe aliquot. We give them 5 cc more than ordered by the physician for waste in the tubing.

At my place, we made a Neonatal blood order set. One of the questions for the physician is #ml to be transfused. This comes into SoftBank. Then when we divide the unit to make our syringe, we put in the ml ordered by the physician. It then prints on the Transfusion Record. Currently it does not print on the ISBT label; Soft knows this is a problem and is working on it.
