How are other hospitals handling competency assessment checklist? Would anyone be willing to share their form? Our current form does not address all 6 measures properly. Thanks

At Northside Hospital in Atlanta, we have a page with each question on it and then following this, we have all the elements to aanswer the question: the Policy or Procedure, the evidence (worksheets, reports, LJ Charts, etc). It is a pain to set up, but after you have it, changes are easily handled . You can get as fancy as you want with the format.

Joint Commission dinged us for our competency compliance at our last inspection so I created a form with each type of competency listed in the standard and each tech has one page for every test or instrument that we use. So we have one form for each chemistry analyzer, hematology analyzer, etc and then a separate form for all other tests not done on an analyzer. We haven't had another inspection since then but they approved our corrective evidence of standards compliance when responding to the survey findings. Not terribly difficult for the lab manager but it does take some time for the techs. Good luck to you!
