Do you use the Buffered Gel card for immediate spin crossmatch [instead of tube]?

If so, what hospital/lab group are you from.

Doing research for our technical advisory team.

As we are a Reference Laboratory, any cross-match we perform involves an atypical alloantibody or an auto-antibody, and so we never perform an immediate spin cross-match.

We use tube IS crossmatched. We have had trouble with the buffered cards in the past supressing antibodies, particularly Kell so we do not use them.

Yes we use buffered gel cards on the ProVue for ISXM and have for about 6 years. Use tube ISXM for traumas and massive bleeders.

I must not be understanding the question...what has the buffer cards and performing the IS crossmatch in gel got to do with detecting Anti-Kell?

We have seen instances where the K antibody that we use as part of our QC for panels did not react as expected when using the buffered cards. We finally completely quit using buffered cards.

immediate spin crossmatch is done in only in case of emergency when you do not have time to do a complete compatibility testing and the objective is to rule out ABO incompatibility. Practically a lot of time is saved doing it in tube instead of using gel card. Kindly anyone will explain the logic / scientific reasoning of using gel card under these circumstances. I would be much obliged.

In the US immediate spin crossmatch is not limited to emergencies. It could be used for any patient with a negative antibody screen and no history of significant antibodies. An antiglobulin crossmatch is required only if the above conditions are not met.
