Our current crossmatch procedure requires that if we have reactivity at IS that is resolved with saline replacement (rouleaux) that we need to set up the tubes again and take testing through AHG phase. If the patient has no history of clinically significant antibody and current antibody screen is negative is an AHG phase crossmatch necessary when rouleaux is detected? Would like to modify this SOP.

That is between you and your Blood Bank Director . . . I don't see why you could not change yor policy.

Every where that I have worked just do the saline replacement. No AHG XM required. I would say if the antibody screen is negative and there is no history of clinically significant antibodies you should be okay.

same here. If you are able to resolve the rouleaux by saline replacement then there is no need to do any additional testing as long as patient's current screen is negative, and no history of antibody.

I agree with all above, but some of my crosstrained techs still get nervous and want to do the AHG crossmatch, so I leave that option open.

Those who are using electronic crossmatch do not even see this scenario, right? I work in a place like yours.
