Just a thought on the original splitting a unit question. We no longer make attempts to split RBC units either - haven't even been asked to in years. But I wrote my policy to have my techs look through our units and pick the smallest ones for a pt that might need slower blood delivery. The units vary from 280 ml to 400 ml, so there is quite a bit of variation to choose from. We weigh every unit anyway (RNs wanted volumes in) so we are used to seeing this variation in units.
