CAP Transfusion Medicine Checklist TRM.41475---requires that information regarding adverse reactions to transfusion must be given to patients that will not be observed by medical personnel post transfusion. We have tried having our nursing staff hand out a sheet with this information when the patients are discharged. The information also incudes physician name and contact information. However, we have difficulty getting our nursing staff to comply. I am interesed in how other facilities are handling this. I would appreciate any suggestions. Thanks!

We include the data in the patient consent form. It is bullet pointed and written in bold. The patient keeps a copy of the consent form. That way the patient 'has' to see it :)
