I got it from my supervisor who got it from Vickie Karavanich in the Lab at Bay Area Hospital in Coos Bay, Oregon. She said she got it via Oregon Health Sciences University but that it came from Sacred Heart (I assume in Eugene Ore). If someone there claims it and provides references please post them. :)
