We have yet to go to 5 day thawed plasma, and my pathologist always ask me what the bigger institutions are doing. So who out there is using 5 day thawed?

The Ohio State University Medical Center uses 5 day thawed plasma routinely.

We use 5 day, in a 400-bed Medical Center.

5 day plasma, 425 bed Level II trauma center. Has saved us tons of $$ due to less wastage and works great for our patient population

We use a mixture of 24 hour and 5 day plasma in a 250 bed level III trauma center. We are in Ohio.

We use both 24 hour plasma and 5 day plasma. We are a 500+ bed hospital

in the state of Ohio.

500+ Teaching Hospital (multi-organ Transplants) in the Nations Capital and have used 5-day thawed Plasma for over 5 years. After switching to 5-day thawed plasma wastage dropped from 40 units per month to about 2 per month.

New Mexico - one trauma hospital, one large tertiary care hospital and 5 regional hospitals all use 5 day thawed plasma. I can't speak for the other hospital system.

It's not as bad as it sounds - we're a remote and rural lab and probably only thaw 2 units a month, if that. Our stock tends to go out of date rather than getting wasted through non-use post thaw. Saying that though we did use 16 units of FFP and 8 units of Cryo the other day in a RTI - cyclist v/s car Cyclist survived - 31 units of blood used also! Thank god he was group A and antibody free...

UMass Memorial Medical Center in Worcester uses 5 day Thawed Plasma only and it has decreased wastage immensely.

Level I trauma center. About 4000 RBC's per year. We use 5-day or 24 hour interchangeably - unless pt has a known factor deficiency (which is hardly ever).

Duke University Hospital (924 acute care beds) uses both 24-hour and 5-day plasma products.

We are a level I trauma center and love it. We use both 24 hour and 5 day. We keep 2 AB thawed at all times along with 2 A's. The AB's are converted right away, then the A's are converted at the end of the 24 hour period. We have seen a huge drop in waste. It is also nice for those orders that are to give then as soon as it is ready, the order is now for hold, or cancelled. :mad:

You ought to at least get him/her to the FDA accepted 6 hour limit - they have had that for years (centuries!?!). If you are in the States - you could also apply to the FDA for a variance to go to 24 hour expiration (the AABB site has an example letter). Even with very low use, that would get you in line with most other hospitals. That said, we also use a mixture of 24 hour FFPTH and 5 day plasma (converted to an additional 4 days after 24 hours, if not used). Works well and has limited our loss too. We use about 40-60 units a month.
