Hello all. We use the Immucor Fetal Bleed Screening Test. The package insert says for the collection of the sample:

"it is best to wait about an hour after delivery...but the sample should be collected as soon as possible thereafter".

Our OB dept wants us to use the post delivery CBC sample for this test, which is sometimes drawn 24 hours after delivery...seems like that is outside the recommendations of the insert, although it is rather vague.

What timeframe does everyone else use? And does anyone know of a reference that recommends a more detailed timeframe for this test?

We draw with the post-delivery H&H. This has been my experience since this test was initiated . . . I have never seen a sensitization.

I just got the Orhto Fetalscreen II kit and noticed the instructions don't set us up for those impossible limits than no one really followed adequately.

Here was my logic why not to worry about it when I used Immucor: they were worried that ABO incompatible fetal cells would be destroyed by the mothers immune system by the time the sample was drawn if we didn't draw it quickly, however, back in the days before RhIG, it was observed that Rh neg moms of ABO incompatible babies were much less likely to become sensitized and make anti-D. Thus I concluded that those that destroyed the RBCs before the sample was drawn were less likely to make anti-D anyway. Again, we haven't exactly seen patients showing up with a lot of anti-D so what we are doing is probably working ok.

Our protocol states that we draw the Fetal Screen about 1 hour after delivery or as soon as possible. On day shift we can hit 'about 1 hour' most of the time and we have to settle for 'as soon as possilbe' on evenings and nites most of the time. The timing is usually related to when the OB service gets around to getting the cord blood specimen ready for pickup rather than our ability to get the cord blood panel done.

Hi,

We collect our samples as soon as possible after the first hour post delivery. We also use Immucor Fetalscreen. We didn't give ou OB department a choice!

Hmmmm - our procedure doesn't specify a post-partum collection time - only that the specimen is stable 24hrs if refrigerated. I think my BB super is going to ban me from this site!!! I keep questioning things (yes, a GOOD thing). Adding THIS issue to my list!!!!

I had someone ask me recently if he could use the the pre-delivery "hold" specimen for the Fetal Bleed Screen...... This was not a new tech. Maybe I better check my procedure.
