I would greatly appreciate feedback on the following:

Cord Blood workup was performed; type A POS, DAT-Pos.

Immucor Elu Kit Results; A1-1+, B-1+, SCI-4+, SCII-4+.

Mom is O POS with Passive D (PD)from Rhogam.

We assumed the the positive Screen Cells was due to PD, however we got

pannaglutination-4+ rxns in the Antibody ID. Could that be due to mom's medication?

What were the results of your last wash? Do you use tubes, gel or solid phase? Does the MOM have a panagglutinin or just the RhIg anti-D? I can't imagine you would get a strong panagglutinin from a marginally positive antibody screen - did the MOM have a positive DAT?

The last wash was negative. We did the DAT in tube and the Eluate in Gel. The mom only had RHIg anti-D, NO Pannagltinnation. DAT on the mom was not done because my boss did not want to do any further investigative.

If it was a panagglutinin you would expect the A1 and B cells to be about the same strength as the panel. I vote to repeat the eluate. I have seen Elu Kit eluates improperly prepared give questionable results. Also, can I assume that the mom's blood type of O pos as stated in the first post is a typo?

I would repeat baby's ABO/Rh type using a heal stick specimen. I would also repeat the DAT and eluate using lavender bullet specimens obtained through heal stick as well. And, as was mentioned, why is an Rh Pos mother receiving RhoGam? Additionally, what meds was the mother administered?

Sorry, that was indeed a typo-Mom is O NEG. According to our records Mom was on Prenatals Vitamins only. I wanted to either repeat the Eluate on the existing specimen or request a new specimen; However my boss thought it better to leave it to the doctor to make his own conclusions.

Our procedures, practice, and most important, reagents do not identify the cause for the weak D; ie low antigen expression or altered antigen expression. When an altered antigen is expressed sensitization and production of an allo-D is possible. This is why RhoGam is administered.
