Hello All :)

This is a question for those of you that provide transfusion for cord blood transplants.

I have a patient that is currently being evaluated for a transplant. She has not been able to have a Bonemarrow match, so they are moving onto a cord blood search.

My question: Do I need to worry about giving this patient CMV negative and Irradiated products? If so, when should these be started?

Deanna,

Given that this patient is a candidate for a BoneMarrow transplant they are most likely immuncompromised indicating a need for Irradiated/ CMV Negative products, the start of which would be initiated by her Oncologist. If you do not get initiation from her oncologist I would inquire.

Yes! Cord blood transplant is another form of hematopoietic progenitor cell (HPC) transplant utilized for immune reconstitution. Below is standard an guidance from the Foundation for the Accreditation of Cellular Therapy (FACT). More information is available on the FACT website.

STANDARD:

B2.3.4 There shall be 24-hour availability of CMV-appropriate and irradiated blood

products needed for the care of transplant patients.

GUIDANCE:

Those blood components suitable for CMV-negative recipients must be defined in the Standard Operating Procedures (SOPs) by the Clinical Program. If leukocyte-reduction is used to meet this standard, there must be a validated method in place (in the Transfusion Service) to ensure that components are suitably leukocyte-reduced. There must be a procedure in place for procurement ofirradiated blood products as needed for patient care.

I agree! The definition of hematopoietic Stem Cells has undergone considerable revision in the last two decades. The hematopoietic tissue contains cells with long-term and short-term regeneration capacities and committed multipotent, oligopotent, and unipotent progenitors. HSCs constitute 1:10.000 of cells in myeloid tissue.
