When making components and relabeling with ISBT label, how are you documenting the label recheck before issue? Documenting on paper? computer system?

We have a label verify in Safetrace TX, if the computer isn't available then we have a paper log.

I work at a blood center, but we also have our computer system perform the 2nd check. When this is not possible, we have a manual log where 2nd checks are documented. One example where it is not possible: labeling platelet products with the actual yield.

Cerner has a Label Verify function, but we have a log for use in the event the computer is down, which is described in a dowtime SOP.

Boooooooo!!!!! Why doesn't MediTech listen to us and build a label check function into the Make Aliquots, Make Components, and Pool Units routines?????? SO FRUSTRATING!!!! We have to use a manual log which is so NOT a part of the operational flow and is just asking for errors, incomplete documentation, etc.

Could you create a Customer-Defined Screen with queries and have them enter Y for Qrys in the Make Components. It would be putting the cart before the horse but it would be documented.

That is basically what we did. We have it attached to the issue screen so that the final check is performed at issue.

I agree with the rest of you Meditech users. Meditech needs to develop a label review routine.

We have a second individual perform a visual inspection of the generated label and compare it to the original label. Sunquest label check will only verify the ABO/Rh and expiration date/time of the generated label, but cannot verify if the rest of the label is correct.
