We use a separate blood bank armbanding system similar to Typenex. My question do you have 2 people ID the patient prior to placing the Blood Bank armband on the patient and if you do how do you document the identities of the two identifiers?

We document the two identifiers by using a sheet in our blood bank kit. It stays with the order for blood products and has areas for the patient ID label, date and time drawn, transfusion history, pregnancy history, etc. There is an area for the two identifiers that verifies the patients identity. This system works well for us as well as CAP.

Thanks for you help. Do you keep all this paperwork in a manual form or do you also have it documented in a computer system?

Its a manual form that happens to be bright orange. If the two witnesses are not filled out the specimen is rejected. We have been using this system for over 5 years and it works great.

Would you be willing to share a copy of your form?

We do the same.
