Dear Colleagues:

We are optimizing our Proficiency Testing Program on Immunohematology in Chile.

The model more apropiate for us, is the put for CLIA regulation, for CAP or AABB accreditation.

Any of you, have a Proficiency Testing Data Sheet, that help me like a model for perform the evaluation objetively?

Not sure I am understanding what you need. Perhaps something is lost in the translation? As far as Proficiency testing, we suscribe to CAP and I have an Xcel sheet that I use to make sure that everyone has had a chance to participate each year. I do have a procedure I would be willing to share. Let me know if that is what you are needing.

My organization is a public service, that offer a Proficiency Testing Program to 100% of Public Blood Services, but, the evaluation system is not the best. So, if we have a sample copy of data sheet, report form... that IM laboratories send with responses, we can establish a model for performing the most objetive evaluation

I think I may have some blank forms around here somewhere. i usually just submit my results on line so I have some hard copies of the result forms that are blank. I will have to send them from home, as I cannot scan at work. Stay tuned!

This year our Laboratory was signed to UK NEQAS. but, we need a example of data entry sheet of EQA that meets the CLIA requirements.
