Looking for a sample HIV lookback letter to a recipient. Could not find one online.

Have your medical director "pen" one. It is going to come from him/her . . . you just need the basics - you rec'd a unit of blood. The blood supplier has determined/discovered that the donor now has a positive test for . . . ; we would like you to come in to be evaluated based on these findings.

You will have to determine if you are going to charge the patient for any testing performed (if you do the testing). My experience is that patient's do not receive this information well. Even donors do not receive these letters well . . . when we sent a notice to donors that their ALT was too high, all they ever saw was that " At the time of your recent donation . . . " they never read past that - they knew they were dying from AIDS. Anyway, it is prudent to have your Medical Director call the patient prior to mailing the letter so that they can receive it in a rational state of mind.
