I'm wondering if there are any organizations that make public their list of best practices in such areas as C/T ratios, blood product wastage and other related topics. Where might comparable data be found on such issues?

I don't have any myself, but it may be worth your while putting "Blood Stocks Management Scheme" into your search engine. This is the scheme run by the National Health Service Blood and Transplant (NHSBT) in England. Of course, the figures only refer to England (and a bit of North Wales), but ther may be some stuff on there that could be useful to you.

This is a great idea. I've wanted to develop a resource to store this info, I think I'll start it soon.

I'll need some volunteers to help develop this tool. I'll post a blog article about it now, and hopefully get some people to help.
