Does anyone keep a log of problems encountered with their bloodbank software? For example, the log could consist of a description of the problem, corrective action, resolution and supervisory review. If not a log, how do you track computer problems? Or do you try to track them at all?

We don't keep a computer problem log; if we have problems with our computer system, we just refer it to our LIS people who either resolve the issue, educate us on our mistakes, or contact Cerner to get the problem resolved.

We do not specifically keep a computer problem log in blood bank. The IS people keep a log of problems and resolutions. I do have computer problems that show up in my event reporting system if they cause a complaint from a customer or a problem that could endanger a patient.

Our Hemocare system maintains a list of triggered errors which can be exported... (lists user, error type, time...)

Additionally - Mediware maintains a database of all problems... in the past they have been nice and mailed a list of our call log, problems and solution... when we asked for it.
