Hello,

We are in the process of redesigning our patient result reports (i.e. result reports that will be placed in the patient's medical record). We are interested in placing a "Confidentially Statement" at the bottom of our report.

Does anyone have an example of a statement that they are willing to share?

We consider the entire patient chart and all included reports to be confidential. No special statement is included.

We also consider our entire "chart"/"patient record" as a confidential document. Our Policies and Procedures in the Hosptial HIPPA program details this so we don't add a separate statement on our reports. We do have a statement to the confidentially of documents that we fax. This is a standard FAX cover sheet and is automatically generated by our LIS when we fax reports.
