For the AABB BBTS 34th addition, has anyone been able to implement any Risk Assessment policies or forms that they would be willing to share?  

I would recommend adding "Risk Assessment" and "Risk Mitigation" as new sections in your Variance tracking system that must be completed by the reviewer for each variance documented.  You should also add it to any of your internal audit report forms, equipment corrective action forms, and proficiency testing result review reports.

That's a good suggestion sgoertzen. I have been attempting to comment on risk as it relates to sample/rgnt/patient testing/product quality in the summary of our incident reports, but that does sound a lot more thorough.  I really would rather not complete the entire form and score (as int he commendable practice) for every occurrence or deviation.  I get the intent though...
