    We check it off on the Maintenance log and document review on the log.  Since you cannot run the Echo unless the QC passes and the QC is on the instrument or archive disks, we never felt the need to print off the paper and keep it.

    We dropped when we switched to Epic Beaker for the LIS.  It has the scanning capability for positive patient ID and bedside label printing.  We also need two samples collected at separate times before giving type specific red cells,  We were the last health system (out of a total of 5) in our area to stop using a separate Blood Bank band.

    All babies born to Rh Neg moms get an ABORh and DAT IgG.  We still have some pediatricians who order on O pos moms.  If the DAT IgG is pos, we do an eluate only if the physician requests.  And they don't request.

    We report the positive antibody screen and add a chartable comment that the patient demonstrates anti-D and it may be due to Rh Immune Globulin given (date) at (facility).  Our LIS will allow us to do an electronic crossmatch in the future when her antibody screen returns to neg.

    We create a unique patient in our LIS  (currently Cerner Millennium) for each CAP sample and place the barcoded label on the sample and run on the Echo.  Results cross to Cerner just like a patient.  Since CAP says to treat like a patient, that is how we test out patients.
