Migrating Patient Test Records
We use HCLL with Epic. Our instrumentation is interfaced to HCLL (now WellSky). For manual testing (tube, gel) reactions are entered directly into HCLL - tubes or gel card must be in hand when recording. We do use paper for downtime and of course recording our antibody panel reactions. There are a few tests that are wonky in HCLL that are easier to record on paper. We also utilize the APBC (which backs up every minute) for when we are down.

Separate Blood Bank Armbands
We dropped when we switched to Epic Beaker for the LIS. It has the scanning capability for positive patient ID and bedside label printing. We also need two samples collected at separate times before giving type specific red cells, We were the last health system (out of a total of 5) in our area to stop using a separate Blood Bank band.

I wish they still made the Immufuge. We use a Hettich centrifuge with a head you can remove and inserts that will hold the tubes in place. But the head is heavy so not as easy to decant as the Immufuge was. But we seldom wash manually.

Automation Daily QC Documentation
We check it off on the Maintenance log and document review on the log. Since you cannot run the Echo unless the QC passes and the QC is on the instrument or archive disks, we never felt the need to print off the paper and keep it.

Routine Testing on Newborns
All babies born to Rh Neg moms get an ABORh and DAT IgG. We still have some pediatricians who order on O pos moms. If the DAT IgG is pos, we do an eluate only if the physician requests. And they don't request.

Reporting prophylactic anti-D
We report the positive antibody screen and add a chartable comment that the patient demonstrates anti-D and it may be due to Rh Immune Globulin given (date) at (facility). Our LIS will allow us to do an electronic crossmatch in the future when her antibody screen returns to neg.

Alarm Activation checks for TempTrak central monitoring system
TempTrak is really a temp recording system and not suited for Blood Bank alarms. We have had the server not record temps so all blood storage refrigerators and freezers have a chart wheel. It does come in handy as a backup when the chart wheel is changed but the tech forgets to push the button to get the pen to record the current temp. We use the manual alarm on the refrigerators or freezers to check the alarms.

Epic Transfusion Administration System?
We have been using Epic BPAM since December 2014 with Cerner which required a workaround to get units to go to Epic. We are moving to Epic Beaker and Mediware HCLL in October 2017. We do have an experienced Blood Banker on the LIS team but any tips in the build would be appreciated. The Beaker build has started but the HCLL build starts in October 2016.

Electronic Crossmatches and CAP
We create a unique patient in our LIS (currently Cerner Millennium) for each CAP sample and place the barcoded label on the sample and run on the Echo. Results cross to Cerner just like a patient. Since CAP says to treat like a patient, that is how we test out patients.

FRIDGES!
We got our first one in 2010 and purchased two more in 2012 to replace our double-door Jewett. We had a few issues with the chart recorder on one of the 2012 fridges but otherwise we have been happy with them. Our maintenance department encouraged us to go with Follett.
