    Here's the official lingo from my policy:
    PRINCIPLE: The Blood Bank performs correlation of tests methods to evaluate the results of the same test performed with different methodologies. Methods in Blood Bank may not correlate 100% due to sensitivity and specificity of each test method. The goal of correlation of methods is not to achieve perfect correlation, but to understand the sensitivities, specificities, and limitations of the different methods used in Blood Bank. Some methods (e.g. Tango and LISS antibody screen) are not used interchangeably, but comparatively, when interfering substances (rouleaux, cold agglutinins, etc) may interfere with a particular method. No method in Blood Bank will always detect all clinically significant antibodies.

    When I played for a bit at being boss I gave them 3 chances - first was a reprint of the paperwork to fill in, the second was a letter that went to the charge nurse, 3rd went to ward manager and then a letter went to the chief exec - it was fab having the chief exec on board as he would give them a rocket. We went 3 years with 100% compliance!

    I would also push for making this a nursing quality/compliance project. Don't let it become your burden.

    We performed a daily review on all returned transfusion record carbon copies and any missing information was highlighted on a copy. This was sent as an official occurrence to the nurse manager for follow-up. It took a little over of a year of this before we consistently received over 95% compliance.

    If the information is required, then it is required.
    If the nurses can't be bothered to fill it in, go down the disciplinary route.
    If the information is NOT required, then ditch that bit of the form.

    We had a patient with anti-Lub.  We gave phenotypically similar units positive for Lub.

    You could also find out if the patient has siblings and test them for compatibility and then have them do directed donations for the patient.

    We have an Anti-Jk3 and an Anti-AnWj.  Both are just so much fun.

    Has everyone completed their IQCPs for micro lab but us? I'm surprised it is not a hot topic for micro!
    Would anyone participate if I threw out a question or two on how certain IQCPs were handled for micro?

    “Select” expired panel cells shall only be used to assist in ID & not used for ID alone. They are “rare” and can be used for pt. testing up to 60 days past exp. date. They shall be QC’d w/ a pos and neg control for the antibody one is trying to rule out. (If trying to rule out an anti E, use E antisera & saline). This will ensure random agglutination is not occurring. If the cells are discolored (brown or green) or appear cloudy, or appear to be contaminated or degraded in any way they shall not be used. These panel cell lot numbers and exp. dates, control lot numbers and exp. dates, and results shall be documented
