Do the Blood Bank refrigerator and freezer have to be externally monitored outside of the blood bank? The lab is staffed 24/7 and able to respond to the freezer or refrigerator alarm. Does security or the Emergency Department have to also externally monitor these alarms? Are there any CAP or AABB standards, or does the FDA require this? Just wondering since it seems to be redundant, and we have an older freezer which may be difficult to hook up.

We do not use potentiators during a prewarmm.

Do anyone require a current sample and a separate blood bank armband for transfusion of platelets? My current Blood bank only requires two historic ABO/Rh tests before transfusing platelets. I am wondering if this meets the CAP standard for prevention of misidentification, TRM30550.

Does anyone interface with an ECHO or Provue for the JAT to result the CAP exm survey?

I work at a multi hospital network and we currently do not participate in the CAP electronic XM on the JAT or J. I am not sure how to prevent other Transfusion Services in our network from "seeing" the CAP eXM results of other hospitals. How do other multi-hospital networks participate in CAP eXm without inadvertently sharing the CAP results? We use HCLL as our LIS system in the Transfusion Services and interface with EPIC/Beaker for our lab LIS.

After the surgery, the specimen outdates after the next 3 full days ( or our time limit of ten full days from the draw date if earlier). If the patient is still in house and the specimen outdates, do you use another armband number (new armband) or the same armband number? We have always used the same armband number because our LIS (HCLL) will not allow testing on an expired specimen, but will allow testing on a new specimen with the same armband #. We do this so we don't have to re-XM on a new specimen when the older specimen is still indated (For example on the tenth full day).

the questions we ask include has the patient been transfused or pregnant in the last 3 months. We extend our PAT specimens 10 days. we place the armband and instructions in an envelope for the admitting nurse to attach the blood bank armband when the patient is identified for the in-patient hospital armband. If the envelope is not available at the time of admission, we start over with a new sample. The questions are asked by the phlebotomist at the time of draw. Does anyone repeat the question concerning pregnancy or transfusions within the last 3 months at the time of in-patient admission?
