Rhogam Reaction
I had a patient in OB who had a reaction to a Rhogam shot that was issued by our transfusion services. Does anyone know how this is treated. Is it a transfusion reaction by the laboratory or a drug reaction by the pharmacy? Does the lab have to report anything or are there forms to be filled out for this? Any ideas will help.

Rhogams with Type&Screen.
I occassionally get confused with this, and will need some help from the experts to clarify my doubt. We had a patient who had a type and screen done and received rogham. On day 1 of the screen the patient delivered, and on day 2 of the screen, a post rhogam was ordered with a fetal screen. Since the screen is still within the last 72 hours, do we still need to do another type and screen, or do we only perform the fetal screen and issue the rhogam. If we have to do another screen considering the other isnt past the 72 hours window, why?

Blood Bank Error
I had an employee who typed a patient wrong, resulting to transfusing the wrong unit. Luckily it was a near miss, because the patient was A pos, but typed as A neg, and transfused A neg units. We have confirmed and changed the patients type to A pos, and are currently updating our policy to make sure that all retypes on patients without history are done before transfusion. Since this wasnt vice versa, or a different type, and the patient wasnt hurt, is there any agency I have to report this to? Also any suggestions on how to deal with perfomance especially if this is the second time the employee has made the same mistake. Very competent employee by the way.

Hello All. I had wanted to discontinue an old procedure we had, where the nurses during order entry must indicate if the patient had been pregnant in the last 6 months, or if they have been transfused within the last 90 days. The techs are supposed to perform a full crossmatch when any of those questions are answered yes. I told my techs that they can check the patients history as always and lean on the right side by perfoming a full crossmatch if the do not feel comfortable with the IS only at anytime during a workup. One of the techs, said that she had 2 instances in the past, where a rare antigen did not show up on the screen. As she indicated if she had not done a full crossmatch she would not have know the patient wasnt compatible to the unit since the screen did not catch it. How could that be possible?
