Liz, the Irish Blood Transfusion service have been using 7 day expiry platelets for some time now (both in pools and apheresis) -bacterial screened at production and at day 4 9fresh sample from pack at day 4) - all results must be negative just prior to issue Think there has been some data published - Crummer

Agree with HeatherVaught, in Ireland we do something similar. We also scan the barcode into the Lab Info system for each irradiated product. We took the decision a couple of years ago to irradiate all platelets (pools and Apheresis).

platelet swirl
In rep of Ireland blood centre we always check swirling throughout the production process (pools and Apheresis) and just before issue to hospitals, most hospitals also check and would return if no visible sign of swirling. Non swirling platelets are rejected and discarded to QC lab who do platelet count. Worth noting that most have platelet count within specs.

Disadvantages: Cold antibodies,...... auto antibodies, ........ the Rh/K cards always have a slight mixed field with K + samples. Advantages: probably best high throughput instrument using card / casette technology

Auto IAT weak positive - probable junk! (lovely serological word) eluate negative (depending on eluate procedure) you are probably dealing with something 'other than red cell antibodies'!!!!!!!!!

Antigen Negative or Crossmatch Compatible?
Agreeing with Malcom, generally give M- units if the patients sample shows anti-M @37C, xm compatible the rest -however if Transfusion reaction was suspected with the others -I would request a sample and investigate/ crossmatch before issuing antigen typed blood to hospital. We are in the unusual situation as covering as a hospital blood bank and a reference lab, I worry greatly as to the identification of such antibodies by labs with limited reagents (as in what really is the antibody!)

Get on to supplier -they will generally contact Imelda Bromilow (if you insist) and they will send an 'official' letter with their recommendations. Learned the hard way after inspections, get it written down from the manufacturer. If you decide on your own protocol- you may be asked to show your Validation.

Control Drop Pipettes
unfortunately the health and safety police have banned the use of glass pipettes in Irish Blood Transfusion Labs, though it is not unknown for one to appear periodically- again the bulbs are a bit flaky!!! Malcome, surely everyone knows you are a man of experience!! When working 4 BGRL surely you were only an embryo!
