  1. I am attaching the competency check list that I borrowed from another institution. BB Competency Check List.pdf
  2. My question involves the BB reagent refrigerator. It is my understanding, correct me if I am wrong, that the BB reagent refrig must have an alram system. I have a small reagent refrig and the external temp monitoring/alarm device is quite worthless. What are some suggestions. This one has a small bottle to sit inside the fridge and a wire comes outside and plugs into a temp/alarm device that is attached on the outside of the door. It is a portable alarm system from Cardinal Health. Any reccomendations?

  4. To those who have the pharmacy dispensing the rhig.....how do you handle the dospensing for out patients?
  5. I am looking for a physician authorization form for transfusing the dreaded word "least incompatible" blood due to Warm Auto, HTLA, etc.
  6. Do you rerquire an authorization from the physician to transfuse "Leat Incompatible" blood?
  7. We find just the opposite. Quite a few weak positives in gel that are clean and smooth in the tube. Strange but true.....

  9. And it also says or "designee", without explaining what credentials must be had. So I guess the person in charge of BB, whoever that may be, can be the 'designee" ???

  11. Reviewing the CAP checklist 9-25-2012...TRM.40770....It does not state that any identifiers must be on an emergency released unit of blood, just a label stating Uncrossed. If the patient is a trauma John Doe that has not had time to be registered, how can any identifier be placed on the unit? There would be no MR number yet.

    I am also having a hard time trying to find a reference for the speed and time to centrifuge BB specimens. We use the pink top tubes, as I am sure most do anymore. I would also appreciate anyone pointing me in the right direction. Thank you too!!
  15. REVISED 07/31/2012 States "education equivalent to an associate's degree (or beyond) in a chemical, physical or biological science or medical technology and at least 4 years experience (one in transfusion medicine) under a qualified laboratory director.

    I am looking for a new Cell Washer. I have always used the Sorvall II and have lots of problems with it. The motor doesn't seem to last very long. I have read up on the Helmer and see that a lot of people are not happy with the weekly care. Does anyone have any suggestions on another one? Anyone using one that pre-warms saline? I would appreciate any and all input. Thanks.
  21. Yes...we do at Georgetown Memorial Hospital System in Georgetown, SC, unless the sample is drawn with a mechanical identification system.

  23. negative auto control. This "auto" d did not show prior to previous transfusion 3 months earlier.
  24. I recently had a known Rh positive who received 2 units of Rh pos blood one month prior suddenly show an auto Anti-D and a Ant-JKb.......ran numerous panels with same result. What is going on???
