Since you are a gel user, how you called Ortho about sending it to their reference lab?

when we went to 5.65, you can't print reports from BB you go back to lab. The spreadsheets a problem we found before go live (January) during validations still hasn't been fixed. You can't put in an autologous or directed donor or markerswhile logging in a unit from batch. You log it in then edit the unit to add what is needed. FFP order one place, change expiration another, assign another place, reprint patient cards in another, then go to issue in another, then back to lab to print report. Most places where you hit enter before, it just sits there and you have to click save or ok to proceed. Need more encouragement?

Meditech is not user friendly. I can't think of anything I like. Things that we like we lost in our update to 5.65. Mary

We are currently on Meditech Client Server. Our previous system was Softbank which I liked.

Orders for Rh Immune Globulin
We do KB in Blood Bank. We also order the RhIG in BB & issue it from there after doing the Fetal Screen or KB.

HTLA?
We had a similar patient and initially called it an HTLA. We kept seeing this patient over several years with the same results weaker one time, stronger the next. We got worried we were missing something & sent it to a reference lab which also reported an HTLA. We just got the patient back in the other day and everything is gone. Mary

Reviewing Physician written orders
We had an incident in which an RN checked out blood & gave without looking at the doctors orders which was crossmatch for surgery only. The patient for some reason thought he was to be transfused & kept bugging them as to when was he going to get it. We had another issue in which blood was given & platelets ordered. Another red cells given instead of FFP post CV surgery. Also due to the confusion between FFP & platelts with the nursing staff in which they have ordered FFP instead of pheresis which resulted in us wasting the FFP, we now require a copy of the orders at the time the order is placed in the LIS.

Rhogam/rhophylac must be under pharmacy control
We've had a couple of very deligent JC inspectors and they haven't said anything about us keeping Rhogam in BB. When we do the cord blood and if appropriate fetal screen, we order the # of Rhogam needed in BB, get it ready & call the floor to pick up. I feel this eliminates a potential omission step in the process that might occur if the floor had to turn around & request from Pharmacy. We start our 6 month window with our inspection, so we'll see then. Mary

Rh negative OB samples positive on Echo, but negative with tube/PEG, etc...
We do some of the prenatals, not for the biggest OB group. We do a T&S on all admissions to the L&D in case blood is needed when one crashes and blood is urgently needed. Mary

We do our Rh negative OB's manually by gel due to this problem. We are also seeing two or three times a week all cells 3-4+ positive antibody screens, and all cells 3-4+ pos on the panel. Now instead of doing a panel on the Echo, we repeat the screen with gel and if negative report that. During go live we saw this on a patient with a history of Anti-E. Our specialist called it into Tech Service to see if they wanted the sample and was told it was a sample problem and not an instrument problem. When I told them I was recommending to my Techs to repeat by an alternate method, he told me that it was a good idea. Mary

Like I said, I think she wanted something to write up. She had an U of Alabama sticker on her name tag with their national championship. I had my Auburn lanyard on.

No nurses don't wear gloves when picking up blood. I think they couldn't find anything, so that's what we got gigged on.

We were sited in a state follow up ipost JC inspection for not wearing gloves when we got a unit of blood out of the refrigerator to issue to a nurse. We had taken our dirty gloves off before doing this. Does anyone know of any regulations requiring this or not requiring? Any help would be appreciated. Thanks Mary

We used to have Soft before going to Meditech. I would highly recommend Soft.

Meditech 5.6 outpatient to inpatient
We are on 5.64 and pre admit with the inpatient number. We have done this for years and have not had a problem with billing & reimbursement. We have a survey the patient fills out and if no pregnancy or transfusion last 3 months will use the sample 7 days.

Oh we've also had problems with the lid breaking off our Clay Adams identical to KKidd.

Ordering a New Centrifuge/Sero-fuge
We have two Clay Adams that neither have worked right since we got them. They'll take a notiion to spin out your cell suspension into the lid even though balanced correctly. We have a Hettich from Helmer that we really like and have bought several more for our chemistry lab.

Refrigerators in Open Heart O.R.
We send large igloo coolers up to our surgery unit. We have a refrigerator in the unit where they keep blood after surgery until the next morning and returned. Due to an incidence in which a unit was spiked, we have bins in the refrig and the blood is put in zip lock bags and sealed before going in the cooler. We had a refrig in general surgery, but due to an anesthesiologist giving a unit to the wrong patient, compatible type thank goodness, we send coolers up for all cases labeled with the patient's name and dob.

Bar codes on blood bands
We had our forms vendor make up a tube label fwith several barcode for us. I measured the labeling area on our BD tubes and we have the first one that tears off to insert into a wide red PDC braclet and 3 more for our tubes. It cost us pennies compared to the commerical, which at the time wasn't available.

Ortho Gel, well 6
We store our gel cards in the refrigerator and it has cut down on the number of issues such as this. Our sister hospital using the same lot as ours had a similar issue, and it was from a dirty dispensor. It was more pronounce toward the end of the bottle. Apparently some one was documenting cleaning, but wasn't or not cleaning it correctly. Mary

Post rhogam Anti-D
We use the ortho panel cells 5-8 for doing our ABS on labor room admissions (we do a T&S on all admitted). We have a order set only for L&D that they can order this test on Rh negative OB's. We use the gel for it and have a disclaimer that it was not tested for Anti D. If the newborn has a positive DAT, then we do an elution on he cord. We have used the gel method since 1995 and have not had an issue during CAP or JC inspections.

Meditech and instrument interface
We are trying to send our Echo back as it missed several weak antibodies during validation. We are a gel useer. We haven't heard from Immucor in 5 months. We didn't interface with Meditech C/S.
