Meditech - Hard stop during entry of unit to force documentation of visual inspection
to save into a Word document, click PrtScn, send to clipboard, then to put in document, click on spot you want it and right click and paste. On the popup boxes, if you want whole screen with the pop up, hit PrtScn, then cancel, then to put in document, click on spot you want it and right click and paste. You'll have to do some trimming then.

Electronic Quality Control
I have all of our QC and temperatures set up in Meditech esp BB and Micro for yrs now. Later I built some Chemistry. Hematology QC is on the analyzer. We can print them out for an inspection whenever needed.

Cleaning the Helmer Plasma Thawer
Our Helmer bath is around 15 yrs old. The times we've needed a cleaner, we used vinegar. Since our water is limestone here, we switched to DI several yrs back.

Disinfecting Blood Bank Coolers
We clean coolers when they come back from ED or OR. You don't know where they have been, esp if blood was used, and the empty coolers made a trip to the smoking shack on way back to BB. I don't want to be implicated in a surgery infection.

platelet availablility
We are 75 miles from our supplier. We keep 2 to 4 apheresis on hand depending on # of CV surgeries and usage. We rarely outdate any. Our supplier swaps out daily.

Ortho Panel Cells -Quality Assurance
We use a patient with an antibody that was tested with the previous lot before placing a new panel in use. We haven't had any issues with past inspections.

Positive DAT
was the antigen type rechecked on the unit? we've had some antisera that if you didn't let it incubate the full time limit, (example if 5-10 in procedure, and didn't go full 10 min) got negative to wk reactions.

Blood suppliers
Our administration wanted to go to ARC from Lifesouth. When we questioned about returns & found out that we couldn't unless we found another hospital to take it. I figured up how many returns we had in the last year and it would have more than eaten up the savings. I sent those figures in response.

We have a Helmer refrigerator and have been very pleased with it.

Unit segments
We pull at time of issue and keep them with that days sample rack. We hold samples for 19 days. Odd time due to way we extend the date to use pre-op samples and CAP requirement post transfusion.

Continuing Issues with Ortho Gel Reagents and or Equipment
Ortho recently opened a new line for manufacturing. Makes me wonder if they haven't gotten the bugs out yet? We are seeing a lot of ? on B cells and SC3 regardless of lot on the Provue. You can see very tiny specks up the gel.

The replace Xmatch wasn't available at the time. The profile has worked well over the years.

Grifols Erytra
We went from the Echo to a refurb Provue before the Vision came out. We have the option of upgrading to a Vision, but will probably do it before moving into our new hospital and have it validated and ready to go on moving day.

We are having a few problems, especially when the cells used on the bench get older. On the Provue, we are having several of reaction ? reviews

Meditech Emergency Issue & Electronic Crossmatches
When we were converting over to Meditech 15 yrs ago, we found an issue with emergency releases if it ordered the IS Xm and we found out later the patient had an antibody. We reported it to MT and we set up a XM profile that had the IS, gel, and tube Xm in it. All are auto NP except the IS. If we need to result something other than the IS, we NP it and change the NP on one of the other tests to the test result we performed. We can't do electronic Xm with the version we have, but when we upgrade to 5.66, I would think we wouldn't have issues.

Recipient antibody cards
I mail the patient a business size card with our 2 hospital logos, our contact information, patient name, antibody and blood type on it. I also put a warning in the allergy section of our IS to call BB and why. I also document on the BB history side of our Meditech the date card sent and OE warning so we know not to sent another one or put the allergy warning in.

Reporting prophylactic anti-D
We built the 5, 6, 7, 8 Ortho panel cells in Meditech. It has a comment not screened for Anti D and we record the date given. We have that test built for Labor Room as part of an Rh negative panel. If the baby has a positive DAT, an elution is reflexed and we will also do an panel & titer on Mom if indicated.
