YOUR OPINION: Dr. must put signature on all lab tests ordered
Ok, I have been out of the loop for awhile. Does this mean that tests we did automatically (antibody identification on positive antibody screen samples) would require extra documentation of the physician's signature?

It's been awhile. http://www.labce.com/ has been good for basic continued ed online. If you haven't already, keep in touch with your Infectious Disease team at your facility. Read up on what are state reportables for your area. Sounds like you are in great hands with some really good micro folks in your dept. Good luck!

How many mistakes is acceptable?
Going back to the original situation...oh my gosh. Working in the transfusion medicine field requires a certain level of accountibility. It sounds like all the regular channels to help improve the situation from the regular employee's part have been done. How frustrating. Tracking and trending of even small oops! and near misses can really give a better picture how a department is running. It's too bad the big ones (like issuing incompatible blood) are being missed & swept under the table. Obviously this original situation presented is not doing well. As an employee, I would leave if possible. Working for an employer that doesn't share the same morals of quality can be the wrong fit. Asking the employee to do another person's job (event reporting, tracking everyone's mistakes) can create a really negative work environment. I would leave lots of documentation on the exit interview.

BB Exam
I agree. The BB can throw some curveballs. I self studied for a little over a year for it. There is a suggested reading list from ASCP. When I took it, geeze, 5?years ago there was alot of pedigree genetic questions & Rhogam qualification questions. Just a tip on testing...you will get one dry erase board. When you get in, write down all the formulas,etc (that you might forget while under pressure) early so when you need them you have it.
