What to do with cooler validation info re: storage/transport?
I think you might have to have 2 policies or you could pick a time/packing configuration for storage (for example, 2 units have acceptable storage temp for 3 hours) that matches the 3 hour transport time for 8 units. Why remove blood every half hour during the 10 C validation? If it is going between refrigerators it will only be removed once. We also had a problem keeping blood cold enough (6 C) using refrigerated gel packs. We don't use them anymore.

I have sent numerous inquires to their email several months ago and have received no reply. Sounds like a great idea. JB

I agree with BLipkin about FDA potency requirements as there should be no potency requirements in an immediate spin crossmatch. I do think that your immediate spin crossmatch using gel should be validated before switching from tube.

Yes, we still use prewarm without enhancement occasionally. We have an SOP that details when and how to use it. I would retrain the tech using 4 drops LISS ASAP and make sure she is not modifying any other procedure/test. JB

In my search, certified thermometers are much more expensive. Are you sure it certified or just traceable?

You can use a validated cooler if they want to have blood at the bedside for actively bleeding patients . As for nursing making mulitple trips - can you use a pneumatic tube system to transport blood?

Just curious as why you use Safe T Vue and thermometer? Can you wrap the units in refrigerated gel pack? No need to validiate if using Safe T Vues and gel packs are kept in blood refrigerator.

Hi khalidm3, Do you have access to chloroquine diphosphate? If you do there is a procedure in AABB Technical Manual for Dissociation of IgG by Chloroquine for Red Cell Antigen Testing of Red Cells with a Positive DAT.

Just curious as to why the patient care area requested blood without tags? Do they think this is causing a delay in issue or they just don't want to be bothered with paper? Never heard of it.

No, I would not issue with out paperwork. Can you prepare your transfusion tags/paperwork before hand to eliminate any delay?

Autumn, Is it not possible to test the baby's plasma for maternal antibodies? If there is no detectable antibody in baby's plasma you do not have to give antigen negative or crossmatched blood. What does your procedure/policy state? JB

Minor crossmatch with microcolumn agglutination method
Just an FYI, in the US, it is not required to do minor crossmatch for packed red blood cells even if the donor has a known antibody, although many hospitals do not accept these units or prefer to wash before giving to patient (some one correct me if I am wrong). To do minor crossmatch you may have several options: 1. For patient's with positive DAT switch to a less sensitive technique such as tube, however, this will not solve all your problems as many minor crossmatches may still be incompatible. 2. Perform an antibody screen on large volume plasma products for patient's with known positive DAT. 3. Perform minor crossmatch and control (patient cells/patient plasma). If minor crossmatch is stronger than control, do not transfuse. (I am not very comfortable with number #3.) Have you asked other colleagues in China what they do? jb
