  1. That is very interesting. We are using a monoclonal reagent with the clone MH04.
  2. That's what i'm trying to find out. ARC said the donor had 16 previous donations.
  3. Thank you everyone for your quick responses. The unit was destroyed and credited by ARC.
  4. We recently had a group B donor unit from ARC type as AB with our monoclonal anti-A. It appears to be the B(A) phenotype. I read in the Tech Manual how to resolve the discrepancy but I want to make sure it is ok to place the unit on the B shelf. The tech manual was vague about it.
  5. Just make sure you keep the Surgiscreen cells away from light. If you can, rotate the Surgiscreen cells between the shifts. They can easily get contaminated which can cause inconclusive results. At my hospital we keep them cover and on ice when they are at the bench. That has cut down on the number of false positves.
  6. When the nurses received the wrong units, did they file incidents reports? That could be used as part of your documentation. I had an employee whose work was similar to this person, she was gone in less than a month but I had documentation. Question are you a co-worker or a lead?
  7. We almsot got burned with this rule. It involved an anti-S. The tech did not see the pattern with the homozygous S. She called it an anti-M. It was picked up upon review before the patient received any units.:fear:

What is the race of the patient. Some black patients will present with low levels of antibodies that sometimes are not picked up with conventional testing. The destruction can be immediate such as you might have found. I agree reference testing might be the best option especially if the eluate does not show anything. I would also try another method such as polybrene or LIS and increase the reaction time. We have found that the anti-S and anti-C are not always picked up with the Gel method. Don't forget to look at immediate spin reactions after a few days after transfusions to see if there is an anmnestic response.
  9. I like the idea of the platelets being sent in a container. What kind of container do you use and how was it validated?
  10. I would like a copy of your checklist thank you
  11. :confused:We have been trying to test alarm activations and the numbers seem to always be off. We have the Isensix system. Has anyone had any sucess? What kind of temperature monitoring device was used to test the temperature ex. probe thermomter?
  12. At our hospital we accept results for prenatals and OR from reputable labs. We do not accept the chart copies but the actual reports from the labs. Then we will second type the specimen by another tech to make sure the type was put into the computer correctly.
