  1. We are luck another hospital in our group is using Immucor-Gamma, so since we are larger we got their kit and are doing the testing for both hospitals. We repeated all sample we still have. So we ran I-G kit on all samples we had. Those samples over 48 hours old we also ran K-B stain to verify that they are negative. K-B using Sure-teck the sample can be as old as 2 weeks. For those other patients that we no longer have sample on. We are going to meet to discuse what we are going to tell the doctors, ect..
  2. Our policy is simple. If the patient has no history of antibodies and has received 10 or more units of RBC in less then 24 hours then we can issue uncrossmatched blood to the patient. If those units are brought back (like from OR) we will keep them around and if we still have them after about 8 hours we will crossmatch them.
  3. Along the lines with calynn, out Jewett fan would ice over and the drawers would feeze making it very hard to open. This freezer was less then 3 years old and we have discontinued using it. It was that bad.
  4. If the patient is demostrating their anti-A1 then we will give them A1 neg or O units. I know it is not clinically significate, but we need to give Immediate Spin negative units. We would do the same for N, P1 or any other cold antibody. If they are reacting at IS then the crossmatch is incompatable.
  5. We require a 2nd ABO on all patients with no history. We do not require a 2nd sample, but that another tech to the testing (if possible). We also do not charge for the additional testing.
  6. We are using Gel and Ortho Confidence QC kit. We use use Ortho 0.8% cells and dilute the QC kit to 1:20 for our gel testing. We get results from 2+ to 3+ (no 4+ results) Also do you QC the MTS dilutent and run negative controls?

  8. We had Forma Refrigerators and freezers for years. Then we switched to Jewett. We have had MAJOR problems with our Jewett FFP freezer and have bought a Helmer. And are in the process of validating it.

  10. Hey Cliff, Our blood supplier Florida Blood Centers (aka CFBB) also supplies us with 10 packs of pre pooled frozen cryo. I do not know what QC they do, before they ship to us. But we in the Blood Bank do no QC. Boy does it save us time. We still keep about 9 individual cryo for orders less then 10.
  11. Kim, you said "This is the first endeavor for our vendor in a laboratory site." Then I would agree with Cliff to go with an known outside vendor to help validate your system. I do not know if you get FDA inspected (not even sure if you are in the US). But if the FDA can come in you better have miles of paper work since you are the first group to use that system.

  13. We have just finished validating and are now compiling all that data. We also ran Lab tests on pre and post samples. Cryo, FFP tested for factors I , VIII, Plt - Plt Count, RBC - Hgb Hct RBC Morphology, Na, K, RBC Oxyhemoglobin, carboxyhemoglogin, methemoglobin

  20. I agree with CCard. The reps are not going to tell you what is wrong with the system. You could have the perfect system for the blood bank, but that most of the data will not cross over the interface then, what good is it. Your tech's will get frustrated because of the double entry or phone calls because the floor can't see something.
  21. Hospital and Lab are using HBOC Star. (Lab is getting ready to look for a new system). Blood Bank is using Mediware LifeLine
  22. Wondering what systems are used in the Blood Bank, Lab, and Hospital.
  23. The Lab is getting ready to look into new LIS, possibly Mckesson Horizon. We are not sure what system the Blood Bank will go with.
