Beaker Result Entry
We use SafeTraceTx and just went up on Epic. Our Hematology clinic wanted to see strength of reaction on DATs. The techs enter a comment in SafeTraceTx, such as IgG=1+ and that crossed to our former system, Cerner, and also crosses to Beaker.

Transfusion Record forms for Bridge downtime
I never understand that, when Bridge is so much faster than a 2 person check! Our CRNAs wanted to do pre-scan just in case something didn't work right, but once they saw how fast it was, they don't do that. We've been live on Bridge for 4 years as of next Sunday, with SafeTraceTx as our transfusion system. While we don't have traumas or MTP per se, we do have a "downtime" process for recording transfusions, including the 2 person check. Our nurses use I-View in Powerchart for all charting tasks - recording vitals, recording where the transfusion took place, etc. When Bridge isn't used, then all information is recorded electronically in I-View. Information may be recorded on a trauma code flowsheet then entered in I-View.

EMR/Paper Charting of Blood Bank Results
That's what we told our HIMS director. We emphasized that the transfused units are displaying in the EMR. Her concern was does this information impact clinical care or is used to make treatment decisions. We told her that there was a decision to transfuse based on other laboratory results or other clinical reasons. If there is a question of least incompatiblity, then there is discussion between the blood bank and ordering clinician. Thanks for the replies, sometimes it's just nice to know what we are all doing!

EMR/Paper Charting of Blood Bank Results
We have asked our HIMS department if there are any regulations that they are aware of that require the recording of blood bank results in the patient's medical record, be it an EMR or paper chart. Specifically, we were wondering about compatibility testing. It currently prints out on our paper charts from Classic, which we send to HIMS for charting. We will be moving to SafeTraceTx next year. So, several questions for the group: 1. What if any results are being placed on the patient's medical record? 2. Does your facility use a paper chart? EMR? 3. If EMR, what system are you using? AABB Standard - Blood Administration5.26.3
How are sites interpreting this AABB blood adminstration standard now that the items that must be verified before transfusion are actually listed? We were wondering in particaular about verifying special transfusion requirements, product (such as irradiated) or the process (give slowly). Thanks! Missy Ryan
