  1. We are considering using our pneumatic tube to dispense blood one unit at a time. Would anyone want to share their experiences or procedures with me?
  2. The other day, we had an A Negative mom deliver who had received prenatal Rhogam 2 months ago. It was still circulating because we had a type and screen on her and the anti-D was still in her plasma (assuming it was the Rhogam). On testing the cord blood, the baby was A Positive with a positive direct coombs. Although it is not our ordinary procedure, an eluate was performed on the cord which was identified as anti-D. Any thoughts?

  5. Does anyone know if the FDA is still inspecting Transfusion Services every 2 years? That was the cycle we have been on for years, but it has bee 2 years and 3 months since they have been here.

    Thanks for the info. I am having trouble finding labels that say "Thawed Plasma" rather than "Thawed Fresh Frozen Plasma". The only one I can find says "Liquid Plasma" (barcode 18501). Any suggestions?

    I am not sure when it happened, but I read about it in the summary of the Ask The Experts questions from the last AABB meeting. I emailed the FDA about it and will give you the info when I get it. The CRF fro a variance is: 21 CFR 640.120.
  9. Precision Dymanic told me that they are making the bands now, but the cards are still being made in Missouri. They will fill orders for card received by 2/11. After that, the cards will not be available for 6-8 weeks while they more the manufacturing of the cards to California.
  10. I would be interested in how many of you are now putting a 6 hour expiration time on thawed FFP.(FDA Reg 21CFR 606 122(m)(3).
  11. Presicion Dymanics is making their own variety of "Hollister" bands and tab sheets, but they are behind in the process. I have received a box of bands and they seem to be equal.
  12. Hollister sold the armband business to Precision Dynamics. I hope your supply is better than mine is, becasue there is going to be a 6 week or more delay on tabsheets and bands.
  13. Does anyone have information about whether or not drawing pretransfusion samples from a line of a patient who is on continuous dialysis affects the testing?
  14. I would be interested in learning how others are handling the new CAP Question TRM.30575 about implementing a system to prevent mistransfusion. Drawing a second sample opens a "can of worms" when that is not possible. Giving O's is not practical, and the mechanical barriers are undeveloped in this country.
  15. We have electronic charting so we can monitor whether or not the completed record of transfusion gets to the patient's chart. We had a programmer write a program which matches component charges to the number of transfusion tags that are on the charts. It was surprising how many were missing. Since this has been an issue on inspections by our State Board of Health, if we find that tags are missing, we send the Blood Bank copy to Health Information for electronic scanning to the chart.

    AB would be best, but it is tough to get. Our supplier has started to supply prepooled cryo(5 units). This is a great time saver.

  18. We don't reconfirm cord blood typings routinely. If we are going to tranfuse the infant, we obtain a heel stick for another typing to rule out a mislabelled (or poorly labelled) cord blood specimen. We have another problem in that the OB department sometimes labels the cord with the mother's name and number.
  19. MJ, I truly sympathize with your ER situation. We went through all of that many years ago (without the mistransfusions, thank goodness), and finally got control back to lab staff or staff trained under our supervision. With all of the new emphasis on patient safety, 100K Lives programs, RFID patient identification, sentinal events, root cause analysis, and so on, when are they going to listen? Maybe if your could expose them to the new "vein to vein" safety, they might listen. Best of luck.

  22. Do you use a patient consent form for derivatives such as RhoGAM and immune globulin?
  23. Each unit will have different start times, vital signs, transfusionist, etc. I think it is best to use single sheets.
