donnawac
Has anyone run into a situation where they receive a lookback notification but could not access the computer system from years ago to be able to find the product history? What did you do? It looks like the hospital migrated all of the patient information but neglected to migrate the product history when they went up on the new system. KKidd
The lookback form from our supplier has a code for records no longer available. I would get input from my medical director for how to proceed.
Maureen
I have gone to our off site storage facility and retrieved paper records. Fortunately it hasn't occurred frequently.
donnawac
There are no paper records to retrieve. This information was only housed in the computer and unfortunately the old hospital IT folks only migrated the patient information into the new system. jalomahe
Is the old system vendor still around? If yes you can check with them to see if they maintained any retrievable records from that time frame (our vendor downloads all records to storage at their home site in case the entire system crashes to the point that all data bases have to be reloaded). If they do you can possibly get them to search the file for you OR you may be able to get them to work with your current IS vendor to migrate the information ... for a price. Otherwise you will need to reply to the lookback as information no longer available.
matt14802
I wouldn't be surprised if your system is on an old server somewhere. Korchek Technologies can get the data off that server and provide you that data in an archive. Who was your old blood bank computer system vendor?
