Verbal Orders

Hello,

Do you accept verbal orders for products?  Is there a regulatory requirement that you have to have written order?  Is there any situations where you accept verbal orders ?

We would only accept a verbal order from theatres - any other situation requires a hard copy.

We accept verbal orders in emergency situations only. We document the all items that get entered into LIS.

We accept verbal orders in emergency situations. The expectation is that these orders will be entered at a later time into the HIS (mostly happens).

The request is documented on one of these forms (attached) and it is stapled to a copy of the checklist that we use during issue of products. This form and checklist are stored with all the other Blood Product Request forms received via the LIS for 10 years.

We have an official verbal order form but it is only used for an initial emergency order (uncrossmatched blood).  We keep it with the paperwork saved on these patients (i.e. the form signed by the Physician to accept uncrossmatched blood; et. al.).  It must be followed up with an electronic order when things calm down (or ASAP).

We accept verbal orders in emergent situations, however we recently configured our LIS system so we could record the order as verbal.  In the background the computer system generates a physician order and a notification that he or she must sign the order.  It solves the issue of policing up physicians and getting them to enter the orders after the fact.  So far it is working great.
