Age of red cells for neonatal exchance transfusions and neonatal OR cases

Hi everyone!

We do neonatal exchange transfusions at my institution and we also like our babies to get babesia negative units.  We have trouble sometimes getting a 5 day old or less unit that is Babesia tested - does anyone do 5-7 day old red cells for exchange transfusions?  The technical manual says 5-7 is ok but I wanted to get other opinions as well and see what has worked for people and their patients.  The 2 more days would help us with getting babesia tested units.

Also - what are some policies for neonates going to the OR and the age of blood they should receive?  We are working on a new policy - leaning towards less than 7 days old but I am curious as to what other do :)

Thanks everyone!! you all are always helpful!!
