Blood Administration Sets

Should there be seperate administration set for RBC,Plasma,Cryo,Platelet prodcuts or could the set be applicable to all products? How often should the administration set be changed? Is there a particular micron filter that is recommended? What is a good resource to find these type of answers?

Infusion Sets

Components must be administered through special IV tubing with a filter designed to remove blood clots and particles potentially harmful to the patient. Standard blood ad- ministration tubing has a 170- to 260-micron filter. The tubing can be rinsed or primed with either 0.9% sodium chloride or the com- ponent itself. The manufacturer’s instructions should be reviewed for proper use. The IV setup should have a mechanism that allows bypass of the blood IV administration tubing to start 0.9% sodium chloride in the event of a

reaction. A suggested mechanism is to have a "Y" port or three-way stopcock close to the infusion site that would allow the administra- tion of 0.9% sodium chloride.

The same set cannot be used to transfuse all components. Different sets must be used, for e.g. if RBCs were transfused with one set the same cannot be used in tandem to transfuse Plts. or else all the plts will be trapped in the filter. Usually the sets are changed after 3-4 units are transfused.
