Transfusion tag's REQUIRED information

Is anyone aware of a current or forthcoming standard or requirement that states:" the expiration date/time of the blood product being transfused SHALL be on the transfusion tag/label as well as the blood product itself"? It is a question that came up while we are in the process of revising some of our forms. Any comments are welcome. In a side note, this info may be beneficial to the transfusionist while performing bedside checks, but is it necessary?

I think that if the transfusion tag is going to become part of the Medical Record the exp date of the component should be part of the info on the tag. I use compatibility labels, which remain attached to the unit, but are discarded at completion of the infusion . . . with no exp date.

From the AABB Standards:

5.19.4 Administration of Blood and Components

Immediately before transfusion, the transfusionist shall verify that all information matching the blood or component with the intended recipient has been verified in the presence of the recipient, item by item.

Verification shall include:

1. The recipient’s two independent identifiers, ABO group, and Rh type.

2. The donor unit number, the donor ABO group, and Rh type.

3. The interpretation of crossmatch tests, if performed.

4. Special transfusion requirements.

5. The date and time of issue.

6. Expiration date of the unit.

But it does NOT say it has to be on the transfusion tag. So, yes, it's necessary that the expiration date is checked at bedside, but not that it is required on the form. We've always had it on our form though.

We have tags automatically printed by our BB IS that have the exp date and time as well as all the other pertinant information. These are printed in duplicate: one goes on the back of the blood bag, and one on the transfusion form.

Even before this, when we had handwritten tags, in the stone age, we hand wrote the exp date and time on the tag.

Same here: we handwrote on the expiration date in the "stone age" and now we have tags automatically printed. (duplicate of blood administration profile except on paperboard.)

We get the BloodTrack system we use to generate a Compatibility and Traceability Label (two part). On both are the Unit No, group and expiry date. The compatibility goes into the Prescription & Management Document in the Patient Notes. The Traceability Label comes back to the lab as hard evidence of fating of the units (has to be kept for 30 years under EU legislation).

The expiry date is also on the Compatibility Report - so plenty of places to be seen.

Do you have the 27th Edition? The standards I am reviewing has this as 5.26.3.

Tranfusion Tag requirments are 5.20.1 (27th Edition)
