How do you comply with the std 5.1.6.3 ...labeling process shall include a second check to ensure that affixed label(s) contain(s) the correct unit number, ABO/Rh, ...

for the aliquot prepared in the syringes or for pooled products.

We have a second tech check everything on the aliquot or pooled product. This is documented in our computer system (MISYS) as a unit label check. On our computer system, a test code ULCK (unit label check) is added and resulted by the tech who performed the check. It's resulted with DONE (unit label check done).

We have a second tech do a "label check" and document (initial) on our sterile docking log sheet that the check was done. The tech checks the aliqout label against the "mother" bag.

We do the exact same thing as dcharland above. We are a pediatric hospital so we do LOTS of label checks for all of the components we split, pool and modify.

We also leave the syringe attached to the tubing, that is attached to the product bag (i.e. octoped, platelets, ffp). The entire unit, original bag and attached syringe, is issued to the nurses. The nurse removes the syringe and transfuses at patient bedside. We do not split or pool any products; but get them from our supplier if needed.

How is everyone going to handle this situation (in particular the bar code readability requirement) with ISBT128? Will you have a barcode printer and put that label on the syringe?
