  1. Does anyone have a standard that you use for how long Blood Bank antisera and reagent red cells can be kept out of storage? The manufacturer's inserts for Immucor state to store at 1-10 C when not in use, but nothing in terms of the maximum time refrigerated reagents can be kept at room temp. We rotate our racks every 8 hours. Thanks for your input!

If a unit is issued at, say, 2210 and the specimen (not the unit) will expire at MN, do they have to stop the transfusion at MN or is it okay to finish giving the unit if it isn't quite done by MN? We use MN on day 3 as our crossmatch/specimen expiration (not exactly 72 hours) so theoretically a specimen even in normal circumstances could be over 90 hours hours old at the time of a transfusion if it was drawn very early in the morning. I am convinced that medically it won't "turn into a pumpkin" at MN but what do you think inspectors would say? Do you have a hard and fast policy for this or leave it up to judgment? Is it okay to issue a unit on an expiring specimen as long as issuing occurs before the specimen expires, the infusion will start by the time the specimen expires, or only if the entire infusion can be completed by the time the specimen expires?
  3. What is your hospital's policy on armband expiration for outpatient transfusion? Do you allow them to be drawn one day for testing and transfused the next, even though they are not inpatient? Does your hospital allow patient's to wear their armband from their inpatient stay home to be used for a routine outpatient transfusion the next day? What if they came in through the ER and received blood, would you allow them to have a routine transfusion the next day in outpatient on the specimen from the night before? The patients receiving outpatient transfusions here are typically oncology/chronic anemia patients that come fairly regularly. Any regulation references on this subject would be appreciated!
  4. I'm sure this has been asked before but I cannot find a feed about it. What does everyone use as their expiration for a sample when the patient has NOT been pregnant or transfused? We currently use 10 days but my pathologist is considering changing it to a 3 day expiration of all blood bank tubes regardless of whether they've had an exposure or not. I'm thinking of the ramifications of this with not being able to do our pre-surgical specimens a week in advance and it would really change our workflow. Advice please.
