  1. Does anyone know of a program for training EMTs and Paramedics how to store and transport blood products as well as transfuse safely (which will probably be a rare task for any one individual)? And then how about a process to assess that they are remaining competent? I don't want to administer such a program as I already have a full-time job but would like to point them in the right direction or make clear what it would take to accomplish this. We might consider providing liquid plasma if they can check all of the required boxes. Our Air Ambulances already carry blood products, but they get a lot of opportunity to transfuse and are accustomed to following strict FAA regulations. I think a lot of them are RNs also. This would be ground ambulances.
  2. Has anyone set up a system where the physician documentation authorizing use of uncrossmatched blood is done via the electronic medical record instead of on paper? If yes: is done as part of the blood order, or as a specific physician patient care note or do you have another process? We currently send a form with the uncrossmatched units and the physician needs to sign and return at some point in time (does not need to be done prior to transfusion). When ordering uncrossmatched blood, sometimes an order is placed in the hospital system for units with special order, uncrossmatched. Sometimes it's placed in the hospital system with no special order but blood bank receives a "verbal" to send the blood uncrossmatced. Sometimes it is picked up as part of a trauma activation prior to the patient even being physically in the hospital or identified/registered. We currently use Epic as our hospital information system but have Sunquest as our lab/blood bank information system so feedback pertaining to these systems would be especially helpful but information from any system would be welcome. Thanks in advance for any input you can give.
  3. Currently, we use a paper form (stocked in each Nursing station) that is completed by RN/MD and physically delivered to Blood Bank to obtain release of uncrossmatched blood. It is very fast and is not dependent on patient being admitted to HIS, specifically ER patients. Does your facility/IT department require that an order for Uncrossmatched Blood must be placed in HIS, i.e., to be consistent with some requirement that all orders must be electronically generated? Would anyone construe this requiremen to mean that a paper form may not be used to order uncrossmatched blood? Has anyone been told by their IT department, that the order for Uncrossmatched Blood must be placed in the HIS before blood can be released for transfusion?
