  1. You still need to have all four quadrants of the ISBT 128 blood label. If the 100mm x100mm label does not fit on the aliquot bag (or on an aliquot syringe), you can remove just a portion of the paper backing from the label and let part of the label hang off of the container (or syringe). This is just another option and there is no preferred or recommended option. The label example above is not compliant and it leaves out the two crucial pieces of information required for traceability, which is the DIN and Product Code. Erwin Cabana Technical Manager ICCBBA

If all you are doing is thawing pre-pooled cryo and transfusing it, then you are not required to relabel the product and you only need to cross out the old expiration and hand write the new expiration date/time. You can optionally choose to relabel the product as Thawed Pooled Cryo and bar code the new expiration date/time but this is not required. If you are thawing individual cryo units and then pooling the thawed single units together, then you are required to relabel the product. Erwin Cabana
  3. I would like to provide some clarification about the Donation Identification Number (DIN) and the printing of the check character. Whenever the DIN is printed, it must be accompanied by a check character. The check character is not part of the DIN itself and it is not encoded into the DIN bar code. The check character should be enclosed in a box when printed to distinguish it from the DIN. The flag characters are separate from the check character, and neither are part of the Donation Identification Number itself. To help distinguish them from the actual DIN, the flag characters should be rotated 90° clockwise and the check character enclosed in a box.   4. The ISBT 128 product description codes for Thawed Pooled Cryo products do not differentiate whether the Cryo was pooled prior to freezing or after thawing individual units. The same Thawed Pooled Cryo code applies to both. Both core conditions of "None/XX/refg" and "NS/XX/refg" are acceptable to use. Some people prefer to use NS (not specified) for their pooled cryo products because they feel that the trace amounts of anticoagulant in each individual unit can sum up to an amount that could potentially affect the patient. "None" is also acceptable to use because (theoretically) pooling multiple units of "None" should still result in "None". If your system doesn't allow you to assign the same ISBT 128 product description code to your Thawed Pooled Cryo, you can use "None/XX/refg" for one and "NS/XX/refg" for the other.
