  1. Places I've worked have always used default volumes for random donor platelets, pooled platelets, pre-pooled platelets (by blood provider), cryoprecipitate and red cells. When we get a call from nursing, we always let them know the default value. When relabelling we would also use the default volumes. The major difference is for neonates. These are aliquotted according to the doctor's orders with extra (usually 5 - 10cc, according to your policy) for the transfusion tubing. The final dispensed volume is available from the pump, as stated previously. I've only weighed blood products when I actually worked in a donor center or drawing autologous units.
  2. During an FDA inspection at another facility (2011) we were cited for using a 24 hour outdate. The then current Technical Manual listed this change, but there was no real notification of this change that I was aware of. The FDA inspector was well-versed in this change and we changed our policy to 4 hour outdates for all blood products in a syringe. You can find it in the 16th Edition on page 294 under Aliquoting: "Cellular components stored in syringes have an expiration of 4 hours, but if stored in an FDA-approved transfer bag, the expiration remains the same as that of the "mother" unit.
  3. We are currently updating to SoftBank BB LIS, and were notified recently that EPIC has received FDA approval for its BB software. However, it doesn't appear to be available for use and I don't know whether anyone is doing beta-testing for them. For those looking at SoftBank, we were told it is a stand-alone system in anticipation of our facility jumping on EPIC Beaker. However, all billing and resulting have to go through SoftLab before crossing over to the appropriate HIS destinations. This was a big disappointment when we asked the right questions and learned the correct answer. Yes, it is a separate module, but it is NOT stand-alone. I don't know whether interfaces are possible without SoftLab, so this would be something to explore.
