    Could it be contaminated saline, perhaps?
    I would repeat using freshly prepared bottle of saline.

    Regardless of gender, we do a weak D tube test on any patient who test at 2+ or below with the Ortho gel anti-D.
    If patients are possible weak D, we will transfuse with Rh negative as a precaution.

    Thanks, everyone. I wanted to validate it especially for 2nd/ 3rd shift staff when there is only 1 tech staffed in the blood bank. They could put on the segment, sample then do other things.

    We use an IH-500.  If the reactions in the software have no discrepancies (from previous or within the current results, weak reactions, error codes, etc.) they will autovalidate to our LIS. 
    Our LIS does not autoverify.  Those are always looked at before being released.

    Answer: The date the transfusion reaction workup specimen(s) was collected should be the billed date of service. This includes all services performed in conjunction with the transfusion reaction regardless of date of completion.

    At my previous hospital we would accept and use a historical ABO from another lab but only if it came from within our hospital system.

    It would behoove you to keep it. Reason being, a unit in your care was issued before all required testing was performed. Even if the blood was returned, you need to keep the documentation as to why it was released from your electronic inventory record.
    Now, say you issue a cooler full of an MTP pack and as the nurse is walking away, they cancel the MTP. The nurse returns the cooler, it doesn’t even make it to the floor, and you haven’t sent the slip for the physician’s signature yet. In that case, it could be a little redundant to make them sign a form for a nonexistent MTP, so I would just leave documentation of a variance from SOP document in the event of inspection with the documentation that an emergency release was initiated at the request of the physician and was canceled so you did not send a form.

    We require a signature from the ordering provider if emergency release is requested whether the product was actually transfused or not. We keep the signed form as part of our documentation for the event.

    On p. 563 of AABB Tech Manual 18th edition, it only mentions that titer methods other than "saline AHG 60 minute incubation" in tube may result in higher titers and "should be validated with clinical findings" (see Malcolm's post, above).  So it does not seem to say one cannot use gel or other methods, just that you need to document validation.
    I have always been a bit uncomfortable with identifying an antibody with gel (for a prenatal), then doing the titers in tube.  But then again, I guess it is the comparison of the series of tube titers that they are looking at.

    Our primary vendor is only adding an increase to the normal platelet charges for LVDS so I just have our revenue cycle team update the pricing and still use the P9035 code.  However, our secondary vendor (for platelets only) has multiple billing codes so I will have one for PLATELETS, PHERESIS, LEUKOREDUCED, one for PAS PLATELETS, APHERESIS, LEUKOREDUCED, one for PLATELETS PHERESIS PATHOGEN REDUCED, and one for LARGE VOLUME DELAYED SAMPLING PLATELET all using P9035 except the PRT which is P9073. 
    We irradiate in house so we have a separate procedure code using CPT 86945.  We use that even if our irradiator is down and our vendors are doing the irradiation.  
    HLA matched platelets we use P9052.  You should be able to use that same code for SDP and PRT units.  I didn't see a code specifically for HLA matched PRT last week when I figured out my billing.

    If they are exceeding 4 hours they are running that unit slooooowly. Most of our infusions are complete around the 2 hour mark.

    It is best from an operational standpoint to do the same thing every time.  If you always do a second type on patients without a history, you won't forget to do one.
