I have found most of the standard blood bank reports useless for transfusion review. I don't specifically remember data mashed up and becoming unreadable, but it is difficult to do a proper review with the multi-line reports that they give you. I had my NPR writer develop a transfusion review report for me which pulls all of the data I need in a semi-colon delimited format. I download this report, export it into Excel, and then each transfusion is on its own line with headings that I can sort any way I need. This has been a lifesaver!

sounds like your negative control is really not controlling anything. If you are using antiglobulin reagents then you should be adding them to you negative control and expect a reaction with check cells. If you qc your reagent daily there should be no reason to run a negative control with each test (i.e., you've tested the reagents with a positive and negative system). For my C-3b,-C3d testing I run my A2 cell as the negative control of the antisera; C3 sensitized cells are the pos control, run w the antisera. I do this testing in gel.

We drain/clean our Helmer waterbath weekly per manufacturer's instructions and use Cleanbath. I wear a lab coat and gloves when I do it because I wear a lab coat and gloves when I do anything.

We have Meditech, and I have built charge only tests for all of the reference lab charges. I manually add these once the workup comes back. There are some that aren't billable, like the after hours charge, so we absorb that cost.

Agreed Malcolm. With Meditech, there is a BIG red warning when the transfusion time parameters are exceeded, and some of the nurses following the offender will end the transfusion, while some won't. Some enter a nursing note, and some ignore it. Most of these happen in our outpatient infusion area where the patient is dismissed before those transfusion time parameters are exceeded and they don't have the chart open anymore, but some still occur on the other nursing units. Our outpatient nurse manager will always take care of it when I notify her, but the other units do not.

Nothing has changed for manual result entry (and it must be BU# by the way), but Richard was asking about performing the retype on automation. This requires a specimen barcode as I mentioned.

I think that this is very possible. We have seen some thawed plasma look like egg drop soup after several hours in the refrigerator. If you place it back in the water bath for about 5 minutes, cryoprecipitate will go back in solution and should be fine to transfuse. If it still looks the same after warming, then you shouldn't use it.

Was the inspector referring to the ISBT label or the IT form? If it is the IT form, look at KB article 27704. This will change the number that prints on the card so it matches the ISBT format. I think that may be what was troubling the inspector, although what he said doesn't make sense. I'm just guessing that he heard this previously and didn't remember the correct reason that it troubled him. If that is not the case, then I don't have any suggestions other than to plead your case with CAP after the citation is confirmed.

Oh WOW! You guys are amazing! We did not know this was an option. We've managed to add the visual inspection field (hard stop) under the other mandatory field.

We just added a required customer defined screen that asks if we verified all labeling. Being able to scan it as a double check would be WONDERFUL!
